All GRADUATING STUDENTS are required to come at the REGISTRAR’S OFFICE for application for graduation on the specified dates set for each program.

Each student must bring:
1. SF10 / F137
2. Original PSA Birth Certificate (for verification)
3. Grade evaluation signed by Department Head
4. Passport size picture (white background)
5. Black ballpen
